Crisis, Step-Down & Transition Support

Our crisis, step-down and transition services provide specialist, time-limited support for individuals with learning disabilities, autism, and complex needs who are at risk of placement breakdown, hospital admission, or delayed discharge. We deliver structured, stabilising environments that reduce risk, support recovery, and enable safe progression to longer-term placements. These services are designed to respond at critical points in a person’s care pathway — providing the right level of support, at the right time, to prevent escalation and promote stability. All support is delivered in line with CQC expectations, least restrictive practice, and the principles of the Mental Capacity Act.

Our Model of Care

We provide intensive, highly structured support focused on stabilisation, assessment, and transition planning. Our model is grounded in Positive Behaviour Support (PBS), trauma-informed care, and autism-informed practice, ensuring that behaviours of concern are understood and proactively managed. Each individual receives a clearly defined, outcome-focused support plan with identified goals for transition. Our teams work closely with commissioners, clinical professionals, and multidisciplinary teams to ensure coordinated discharge planning, risk management, and continuity of care. The focus is always on moving individuals forward safely — whether that is returning to the community, transitioning to supported living, or stepping down from more restrictive environments.

Referrals

Referrals are accepted from local authorities, ICBs, and clinical teams. Each referral is reviewed urgently where required, with a structured assessment to ensure we can safely meet the individual’s needs and support a clear transition pathway.

Her career began in early childhood intervention, where she worked closely with children requiring additional educational and developmental support, developing a strong understanding of child development, autism, learning challenges, and family engagement. Over the past four years, Rebecca has played a leadership role overseeing operations for organizations providing early intervention services for children with autism. Working alongside local management teams, she has been involved in operational oversight, staffing, quality assurance, strategic planning, and program development. Rebecca brings a child-centered approach to leadership and is deeply committed to creating supportive environments that help children thrive, grow, and achieve positive long-term outcomes

Ben Steuer

Ben Steuer brings deep expertise in finance, business operations and strategic growth, with extensive experience across care home investments, real estate and private enterprises. Throughout his career, Ben has specialized in financial structuring, acquisition analysis, cash flow management and long-term growth planning, helping organizations build strong financial foundations while navigating complex operational challenges.
