Crisis, Step-Down & Transition Support
Our crisis, step-down and transition services provide specialist, time-limited support for individuals with learning disabilities, autism, and complex needs who are at risk of placement breakdown, hospital admission, or delayed discharge. We deliver structured, stabilising environments that reduce risk, support recovery, and enable safe progression to longer-term placements. These services are designed to respond at critical points in a person’s care pathway — providing the right level of support, at the right time, to prevent escalation and promote stability. All support is delivered in line with CQC expectations, least restrictive practice, and the principles of the Mental Capacity Act.
Our Model of Care
We provide intensive, highly structured support focused on stabilisation, assessment, and transition planning. Our model is grounded in Positive Behaviour Support (PBS), trauma-informed care, and autism-informed practice, ensuring that behaviours of concern are understood and proactively managed.
Each individual receives a clearly defined, outcome-focused support plan with identified goals for transition. Our teams work closely with commissioners, clinical professionals, and multidisciplinary teams to ensure coordinated discharge planning, risk management, and continuity of care. The focus is always on moving individuals forward safely — whether that is returning to the community, transitioning to supported living, or stepping down from more restrictive environments.

Our Specialist Care
Who We Support
Adults (18+) with learning disabilities and/or autism, including individuals with:
- Mental health needs alongside LD/autism
- Complex needs requiring intensive short-term support
- Delayed discharge from inpatient settings
- Recent or potential hospital admission
- Behaviours of concern placing placements at risk
Admission Criteria
- Referrals from local authorities, ICBs or clinical teams
- Clear transition or discharge pathway identified
- Requirement for short-term, intensive support
- Step-down from hospital or more restrictive environments
- Individuals in crisis or at risk of placement breakdown
Exclusion Criteria
- Long-term placements without defined transition goals
- Risks that cannot be safely managed within a community-based setting
- Individuals requiring secure forensic or inpatient psychiatric care
